Chronic conditions (such as the "Three Highs"—high blood pressure, high blood sugar, and high cholesterol—or chronic hepatitis B) often have no obvious symptoms in their early stages. However, as long as you undergo early screening and active management, you can take control of your health! 

The Government has launched the "Cardiovascular Disease Risk Factors and Chronic Hepatitis B Screening and Management" under the " Primary Healthcare Co-Care Network " to provide comprehensive health protection for everyone! We will fully assist you in pairing with a family doctor to receive screening and long-term follow-up services.

 Two Major Focus Areas of the Service:

Cardiovascular Disease Risk Factors Screening and Management

Target Audience: Hong Kong residents aged 45 or above who have not been diagnosed with diabetes or hypertension.

Details: Provides screening services for diabetes and hypertension, as well as blood lipid tests, to comprehensively assess and manage the risks of the "Three Highs." If diagnosed, subsidized consultations and long-term follow-ups will be provided!

 Chronic Hepatitis B Screening and Management

Target Audience: Eligible Hong Kong residents born in or before 1988 whose family members or sexual partners are chronic hepatitis B patients, and who are undiagnosed, asymptomatic, and have not completed the full course of hepatitis B vaccination.

Details: Provides hepatitis B screening (free rapid tests can be arranged) and subsidized doctor consultations/follow-ups for higher-risk individuals.

Practicing "One Person, One Family Doctor": Pair up with a dedicated family doctor to receive continuous, holistic healthcare.

Government Subsidies: Eligible individuals can receive subsidies for screening and medical consultations.

Multi-Disciplinary Team Support: Followed up jointly by a team of family doctors, registered nurses, and allied health professionals to help you set personalized health management goals!
